Waiver, Release of Liability, and Assumption of Risk

Effective Date: 3/23/26

By enrolling a child in services provided by RVG Seattle / Level Up Lab (“Company”), including mobile STEM classes, preschool programs, camps, trailer-based activities, and private parties, you agree to the following:

1. Assumption of Risk

Participation in Level Up Lab activities involves inherent risks, including but not limited to:

• Physical movement and play in a mobile trailer environment

• Use of STEM tools, materials, and equipment

• Interaction with other children

You acknowledge and voluntarily assume all risks, both known and unknown.

2. Release of Liability

To the fullest extent permitted by law, you release and hold harmless RVG Seattle / Level Up Lab, its owners, employees, and contractors from any claims, liabilities, damages, or expenses arising from:

• Injury or illness

• Participation in activities

• Use of equipment or the mobile trailer

3. Medical Authorization

You confirm your child is physically able to participate.

In case of emergency, you authorize staff to:

• Provide basic first aid

• Seek emergency medical care if necessary

You are responsible for any resulting medical expenses.

4. Behavior Policy

Children must follow staff instructions at all times.

Unsafe or disruptive behavior may result in removal from activities without refund.

5. Health & Allergies

You agree to disclose any relevant:

• Allergies

• Medical conditions

• Special needs

While reasonable precautions are taken, a completely risk-free environment cannot be guaranteed.

6. Mobile Trailer Acknowledgment

You understand that activities take place in a mobile STEM trailer, and accept the risks associated with participation in this environment.

7. Photo & Media Release

Unless you notify us otherwise in writing, you grant permission for your child to be photographed or recorded for:

• Marketing

• Social media

• Website content

8. Indemnification

You agree to indemnify and hold harmless RVG Seattle / Level Up Lab from any claims arising from your child’s participation.

9. Agreement

By booking services, registering for a class, or completing a purchase, you confirm that:

• You have read and understand this waiver

• You agree to all terms

• You are the parent or legal guardian of the participating child
